Qualifications

• Graduate of an accredited Nurse Practitioner or Physician Assistant program.
• Current and unrestricted NP or PA license in the applicable state or eligibility for licensure.
• National board certification as a Nurse Practitioner or Physician Assistant.
• DEA registration or eligibility to obtain.
• Minimum of one year of clinical experience preferred; experience in post-acute care, geriatrics, internal medicine, family medicine, hospital medicine, or long-term care strongly preferred.
• Strong clinical assessment, diagnostic, and care coordination skills.
• Excellent communication and interpersonal abilities; ability to work independently while collaborating within a multidisciplinary team.
• Proficiency with EMR systems.

Responsibilities

• Provide patient-centered care in post-acute settings.

We are seeking a dedicatedNurse Practitioner or Physician Assistant to join our growing Post-Acute Careteam Springfied, MA. This full-time position provides an excellent opportunity todeliver high-quality, patient-centered care to individuals transitioning fromacute hospital settings to skilled nursing and rehabilitation facilities.

Providers will workcollaboratively with interdisciplinary teams to manage chronic and acutemedical conditions, support recovery, reduce hospital readmissions, andoptimize patient outcomes in the post-acute setting.

Position Highlights:

• Full-time schedule: Monday –Friday, 9:00 AM – 5:00 PM
• No weekends
• No call responsibilities
• Facility-based rounding inpost-acute care settings
• Autonomy with strongclinical and operational support

Compensation & Benefits:

• Base salary: $120,000annually
• Total earning potential:$180,000 – $220,000+
