Qualifications

  • Experience with EMRs and healthcare technology platforms.

Responsibilities

  • Support clinical and operational initiatives across multiple post-acute care facilities.
  • Collaborate with facility leadership, nursing staff, and healthcare providers to improve workflows and performance.
  • Assist in developing, implementing, and refining clinical and operational processes.
  • Monitor key operational metrics and identify opportunities for improvement.
  • Coordinate transitions of care and ongoing patient management initiatives.

Position Overview

We are seeking an experienced Nurse Practitioner with a strong background in skilled nursing facilities (SNFs) to help oversee and support clinical operations within our growing post-acute care organization. This role is ideal for someone who understands the day-to-day workflow of nursing homes, enjoys building processes, collaborating with interdisciplinary teams, and improving operational efficiency across multiple facilities.

The ideal candidate is highly organized, proactive, and comfortable working both independently and cross-functionally to ensure operational excellence and high-quality patient care.

Responsibilities
  • Support clinical and operational initiatives across multiple post-acute care facilities.

  • Collaborate with facility leadership, nursing staff, and healthcare providers to improve workflows and operational performance.

Preferred Qualifications

  • Background in quality improvement, care coordination, or operational leadership.

  • Familiarity with Medicare and post-acute care reimbursement models.
