Total Extended Care Services in Akron, OH is seeking a dedicated Nurse Practitioner or Physician Assistant to join our PM&R team in sub-acute rehab. You will partner with our PM&R physician and work within an interdisciplinary, specialized team to improve patient outcomes in senior living communities.

This role offers flexible scheduling with full-time or part-time options and an emphasis on collaboration, timely charting, and reducing readmissions through on-site rehab care.

Qualifications

• Licensed Nurse Practitioner (Required) or Certified Nurse Practitioner (Required).
• Licensed Physician Assistant (Required) or Certified Physician Assistant (Required).
• Must be licensed in OH and have reliable transportation.

Responsibilities

• Partner with the facility's therapy department to provide the best possible rehab care.
• Communication and collaboration with the patient's primary care team to ensure the best outcomes.
• Timely charting/documentation is required but can be done from home.

In this role, you will partner with our PM&R physician in the sub-acute rehab setting.
