Job summary

Total Extended Care Services is seeking a dedicated Nurse Practitioner or Physician Assistant to join our PM&R team in a sub-acute rehab setting. You will partner with PM&R physicians, collaborate with therapists and nurses, and help improve patient outcomes in senior living communities.

Location is in-person at skilled nursing facilities with flexible, part-time schedules and a focus on interdisciplinary care. Timely charting and coordination with the primary care team are essential to success.

Qualifications

• Licensed Nurse Practitioner (Required).
• Certified Nurse Practitioner (Required).
• Licensed Physician Assistant (Required).
• Certified Physician Assistant (Required).
• Must be licensed in MI.
• Reliable transportation.

Responsibilities

• Partner with the facility's therapy department to provide rehab care.
• Coordinate with the patient's primary care team to ensure optimal outcomes.
• Timely charting/documentation; may be done from home.
