Job summary

Humana's Conviva Senior Primary Care in Lake Charles, LA is seeking a Nurse Practitioner or Physician Assistant to join a collaborative care team delivering comprehensive, value-based care to seniors. You will work with physicians, pharmacists, care coaches and behavioral health specialists to coordinate high-quality care.

Your Role & Responsibilities

What We're Looking For

• You will have a Master's Degree in Nursing or completion of a PA program with board certification.
• You will have an active, unrestricted NP or PA license in the state of practice.
• If selected for this role, we will require you to screen for TB.
• You’ll report to the Associate Medical Director

Pay Range

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.

$123,800 - $170,300 per year

This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
