Job description

Nurse Practitioner / Physician Assistant
Company Overview

Total Extended Care Services is a growing Physical Medicine and Rehabilitation group dedicated to improving function, comfort, and quality of life for patients in sub-acute rehab and senior living settings. We partner closely with skilled nursing facilities to deliver on-site, specialized rehabilitation and pain management services.

Our team includes Board-Certified PM&R physicians, experienced nurse practitioners, and physician assistants who value collaboration, detailed clinical care, and measurable outcomes for a medically complex, primarily senior population.

The Opportunity

We are seeking a Nurse Practitioner or Physician Assistant to work in-person in skilled nursing facilities, partnering directly with a PM&R physician and the facility therapy department. This role is ideal for an advanced practice provider who wants to build depth in rehabilitation medicine, influence patient outcomes daily, and be part of a stable, team-oriented environment.

You will enjoy a flexible schedule, no on-call responsibilities, and the ability to complete documentation from home, while still having a strong clinical presence with patients and the interdisciplinary team on site.

What You'll Do
  • Provide on-site rehabilitation-focused care in skilled nursing facilities, managing a panel of patients under the guidance of a PM&R physician.
  • Conduct thorough assessments and follow-up visits to optimize function, pain control, and overall rehab progress for sub-acute and long-term residents.
  • Partner closely with physical, occupational, and speech therapists to align treatment plans, adjust goals, and ensure continuity of rehab care.
  • Coordinate care and maintain clear communication with primary care providers, nursing staff, social workers, and families to support safe, effective transitions and reduced rehospitalizations.
  • Complete accurate, timely charting and documentation, with flexibility to perform much of the documentation from home after on-site patient visits.
What Success Looks Like
  • Within your first 6–12 months, you are a trusted clinical partner to the facility therapy department and nursing leadership, consistently identified as a key resource for rehab-related issues.
  • Your patients show measurable improvement in function, pain control, and satisfaction, contributing to better facility rehab and quality metrics.
  • Readmission rates and avoidable transfers to acute care for your patient panel are reduced through proactive management and strong coordination with the broader care team.
  • Documentation is completed accurately and on time, supporting smooth billing, compliance, and communication across the interdisciplinary team.
What We're Looking For
  • Licensed and certified Nurse Practitioner or Physician Assistant with active licensure in Illinois.
  • Comfort working in skilled nursing or senior living environments and an interest in Physical Medicine and Rehabilitation or geriatric care.
  • Strong communication and collaboration skills with physicians, therapists, nurses, social workers, patients, and families.
  • Detail-oriented, reliable, and organized, with dependable transportation and the ability to manage a flexible, outcomes-focused schedule.
Compensation and Benefits
  • 401(k) with company match.
  • Medical, vision, dental, and short-term disability insurance.
  • Flexible schedule with no evenings or weekends on call.
  • Licensure reimbursement.
