Job summary

ComplexCare Solutions is seeking a Nurse Practitioner for per diem work covering Albemarle County, VA. The role involves performing comprehensive in-home and telehealth assessments to support health plans nationwide, documenting findings and coordinating with PCPs and plans to close gaps in care.

The NP will independently assess, develop evidence-based treatment plans, and ensure timely follow-up while maintaining HIPAA compliance and high-quality patient communication in a remote setting.

Qualifications

• Active un-encumbered nursing license and NP certification.
• ANCC or AANP board certification as NP or CNS in Family, Adult, Gerontology or Emergency Medicine.
• Current CPR certification.
• Knowledge of CMS Regulations and NCQA HEDIS Guidelines.

Responsibilities

• Conduct comprehensive home and telehealth assessments with members.
• Document encounters and medical conditions per CMS guidelines.
• Coordinate with PCP and health plans to close care gaps.
• Maintain HIPAA-compliant records and timely follow-ups.
• Provide evidence-based treatment plans for chronic conditions.
