Job summary

One to One Health in Indiana seeks a Board Certified Nurse Practitioner or Certified Physician Assistant to provide direct and indirect patient care in our clinic under supervising physicians. You will evaluate and treat acute and chronic conditions, perform wellness screenings, order tests, and develop treatment plans tailored to patients.

You will work with a multidisciplinary team, communicate clearly with patients and staff, and maintain accurate EMR documentation.

Qualifications

• Board Certified Nurse Practitioner (NP) or Certified Physician Assistant (PA) with an active license.
• 1 year of experience in family medicine, internal medicine, or urgent care.
• DEA license preferred.
• BLS certification required.
• Excellent interpersonal and customer service skills.
• Ability to follow written step-by-step instructions and communicate clearly.

Responsibilities

• Provide direct and indirect patient care under supervising physician.
• Evaluate and treat acute health problems using evidence-based protocols.
• Create and implement appropriate treatment plans.
• Order and analyze diagnostic tests and monitor interventions.
• Provide health education and patient advocacy.
• Enter patient data into the EMR accurately and promptly.
• Collaborate with multidisciplinary team and refer as needed.
