Qualifications

• Completion of an accredited NP or PA program.
• Active Indiana APN/PA license with prescriptive authority.
• Minimum two years of relevant clinical experience.
• BLS/ACLS and DEA certifications; DOT examiner preferred.

Responsibilities

• Evaluate and treat acute health problems.
• Manage chronic conditions using evidence-based protocols.
• Order and analyze diagnostic tests; provide health screenings.
• Enter patient data into the EMR accurately.
• Collaborate with multidisciplinary team and refer as needed.
• Provide patient education and health coaching.
• Ensure care under supervising physician’s protocols.
