Job summary

Hospice of Muskegon County, Inc. in Muskegon, MI is seeking a Nurse Practitioner - Palliative Care to provide direct patient care in collaboration with the Medical Director and involved physicians, while also supporting nursing staff and education.

Independent functioning within the care team is expected. The role includes a weekend on-call rotation and focuses on pain and symptom management, end-of-life care, and education for patients, caregivers, and families within a compassionate palliative

Qualifications

• RN licensed in Michigan.
• Preferred minimum of two years RN, NP, or APN home care, geriatrics or Hospice/Palliative Medicine experience; ACHPN certified or willing to obtain.

Responsibilities

• Provides comprehensive direct patient care in collaboration with Medical Director and physicians; can function independently.
• Maintains patient records and supports sound medical practice and reimbursement.
• Promotes quality palliative care and integration of services across care continuum.
• Assists with pain and symptom management and end-of-life care as needed.
• Participates in ongoing quality initiatives and staff education.
