Job summary

Hospice-of-Michigan, through the NorthStar Care Community, seeks a Nurse Practitioner to lead clinical practice for hospice and palliative care. The role requires delivering direct patient care while providing administrative leadership to support NPs within the region.

Under a collaborative practice agreement with physicians, the NP will participate in on-call duties, contribute to care coordination, and mentor staff. Strong communication and organizational skills are essential.

Qualifications

• Current licensure in the state where serving as a Nurse Practitioner.
• Two years NP hospice/palliative care experience preferred.
• Master’s degree in Nursing.
• Board certification as an Advanced Practice Hospice and Palliative Nurse (ACHPN) preferred.

Responsibilities

• Assumes responsibility and accountability for the NP Clinical Lead role.
• Deliver direct hospice/palliative patient care ~80% of time.
• Provide administrative and leadership responsibilities ~20% of time.
• Complete timely EMR documentation and coordinate care with teams.
• Collaborate with physicians to establish goals of care and ensure continuity of care.
• Participate in on-call schedule and quality improvement activities.
