Our providers deliver comprehensive medical care to residents in Skilled Nursing, Assisted Living, and Independent Living communities. As a Nurse Practitioner or Physician Assistant, you will provide primary and chronic care management services to residents in long-term care communities.

• Communication with nursing staff, families, pharmacies, and other healthcare partners

Compensation

• Up to $125,000 annually with Total Compensation Package: $133,000–$150,000+ annually

Qualifications
