Job summary

HarmonyCares is seeking a Nurse Practitioner to deliver in-home primary care to high-acuity patients in a physician-led, team-based model.

You will focus on patient-centered care, taking the time needed to address complex needs and coordinate with a multidisciplinary team in diverse home health settings.

Qualifications

• Licensed as an NP in the state where HarmonyCares Medical Group offices are located.
• Federal DEA license/eligibility required.
• Geriatric training/experience preferred.
• Active CPR/BLS certification.
• Board Certification for NP.
• Must maintain a valid driver’s license and good driving record.

Responsibilities

• Provide holistic in-home primary care to high-acuity patients.
• Practice medicine with focus on patient care over volume; take time to address patient needs.
• Collaborate with team to coordinate care in home health, geriatrics, hospice and primary care contexts.

The Nurse Practitioner who wants to practice medicine with a focus on patient care, not volume. We want our clinicians to take the time needed to truly address the patient’s needs.
