Job summary

HarmonyCares is seeking a Nurse Practitioner to provide home-based primary care to patients with complex healthcare needs. The clinician will focus on geriatric and palliative-type care, delivering personalized assessments in patients’ homes.

The role emphasizes patient-centered care, not volume, with collaboration across an interdisciplinary team to ensure high-quality outcomes. Strong clinical judgment and a valid NP license are required.

Qualifications

• Licensed as a NP in the State where HarmonyCares Medical Group office located.
• Federal DEA License/eligibility.
• Geriatric training/experience preferred.
• Active CPR/BLS Certification.
• Board Certification.
• Must maintain a valid driver’s license and good driving record.

Responsibilities

• Provide direct patient care in home health, geriatrics, hospice and primary care settings.
• Deliver patient-centered care with time to address each patient’s needs.
• Coordinate with an interdisciplinary team to optimize care for high acuity patients.
