Job summary

HarmonyCares seeks an Advance Practice Provider to manage chronic conditions of a high-risk geriatric patient panel in home settings, Monday through Friday. The role emphasizes patient-centered care delivered in patients’ homes, with a focus on improving quality of life and outcomes.

• Licensed as NP or PA in the state where HarmonyCares Medical Group operates.
• Federal DEA license or eligibility.
• Geriatric training/experience preferred.
• Active CPR/BLS Certification.
• Board Certification.
• Must maintain a valid drivers license and good driving record.

Responsibilities

• Provide advanced practice care in home health, geriatrics, hospice and primary care.
• Take time to address patient needs with a patient-centered approach.
• Deliver care for high-acuity geriatric patients in patients' homes Monday-Friday.

The Advance Practice Providers we seek are those who have directly related experience in home health, geriatrics, hospice and primary care. The Nurse Practitioner who wants to practice medicine with a focus on patient care, not volume. We want our clinicians to take the time needed to truly address the patient’s needs.
