Job summary

HarmonyCares Medical Group is seeking a licensed Nurse Practitioner to deliver in-home primary care to high-acuity patients in Jersey City, New Jersey. You will practice with a patient-focused approach, spending meaningful time with families and coordinating care within a physician-led team.

Responsibilities include comprehensive assessments, creating care plans, and providing home visits Monday through Friday.

Qualifications

• Licensed NP with current state license
• DEA license eligibility
• CPR/BLS certification active
• Board certification in relevant specialty
• Valid driver’s license with clean record
• Geriatric/home health experience preferred

Responsibilities

• Provide in-home primary care to high acuity patients in a home setting.
• Deliver patient-centered care with focus on quality and outcomes.
• Coordinate care within a physician-led team and document visits thoroughly

Responsibilities

The Nurse Practitioners we seek are those who have directly related experience in home health, geriatrics, hospice and primary care. The Nurse Practitioner who wants to practice medicine with a focus on patient care, not volume. We want our clinicians to take the time needed to truly address the patient’s needs.

Qualifications

• Licensed as a NP in the State where HarmonyCares Medical Group office located
• Federal DEA License/eligibility
• Geriatric training/experience preferred
• Active CPR/BLS Certification
• Board Certification
• Must maintain a valid driver’s license and good driving record
