Job summary

HarmonyCares seeks a Nurse Practitioner to deliver in-home primary care to high-acuity patients in supported markets. You will focus on patient-centered assessments, develop care plans, and collaborate with a multi-disciplinary team to improve outcomes.

Ideal candidates hold Virginia NP licensure, DEA eligibility, and board certification, with geriatric experience and CPR/BLS; a driver license is required for home visits, with travel as needed in select markets.

Qualifications

• Licensed as an NP in the state of Virginia or applicable state.
• DEA license eligibility or active DEA registration.
• Board Certification as a Nurse Practitioner.
• Geriatric training or experience preferred.
• CPR/BLS certification current.

Responsibilities

• Provide in-home primary care to high-acuity patients in assigned markets.
• Deliver patient-centered care with time to address needs, not volume.

Responsibilities

The Nurse Practitioners we seek are those who have directly related experience in home health, geriatrics, hospice and primary care. The Nurse Practitioner who wants to practice medicine with a focus on patient care, not volume. We want our clinicians to take the time needed to truly address the patient’s needs.

Qualifications

• Licensed as a NP in the State where HarmonyCares Medical Group office located
• Federal DEA License/eligibility
• Geriatric training/experience preferred
• Active CPR/BLS Certification
• Board Certification
• Must maintain a valid driver’s license and good driving record
