Job summary

Crossroads Treatment Centers in Tyler, TX seeks a Nurse Practitioner to evaluate and manage patients with opioid use disorder in our outpatient clinic. You will prescribe appropriate medications and treatments, complete medical documentation, and collaborate with staff to provide holistic care.

You will perform injections and coordinate with community services to support recovery, in a patient-centered, evidence-based MAT program committed to accessible care.

Qualifications

• Certified Nurse Practitioner in Texas is required.
• Texas DEA licensure required.
• Experience in OTP/SUD treatment or addiction medicine is preferred.
• Additional education or certification may be required.

Responsibilities

• Evaluate and manage patients with opioid use disorder in outpatient clinic.
• Prescribe medications and treatments as indicated by patient assessment.
• Complete medical documentation per Crossroads standards.
• Collaborate with clinic staff to provide holistic care, including referrals to other providers.
• Will perform injections.
