Job summary

Twin Cities Physicians is seeking a full-time Mobile Wound Care Nurse Practitioner (AGNP/CNP) to provide advanced wound care at senior living communities across the Twin Cities Metro area. The role combines clinical leadership with hands-on patient care, focusing on healing and comfort in the home setting.

As an NP, you’ll collaborate with physicians and staff to deliver high-quality, compassionate care, while maintaining dignity and independence for aging adults.

Qualifications

• Active Minnesota Certified Nurse Practitioner (CNP) license.
• AGNP preferred.
• Experience in wound care, geriatrics, primary care, post-acute care, or skilled nursing preferred.
• Valid driver's license and ability to travel throughout the Twin Cities Metro Area.

Responsibilities

• Provide advanced wound care assessments, treatment planning, and ongoing management of complex wounds.
• Conduct routine rounding visits throughout assisted living, memory care, skilled nursing, and long-term care communities.
• Manage medications, diagnostics, treatment plans, and follow-up care.
• Collaborate with physicians, nurses, therapists, caregivers, and community staff.
• Support proactive interventions that improve healing outcomes and reduce avoidable complications.
