Job summary

Twin Cities Physicians is seeking a full-time Mobile Wound Care Nurse Practitioner (AGNP/CNP) to deliver advanced wound care across senior living communities in the Twin Cities Metro. You will assess, plan, and manage complex wounds, collaborating with physicians and care teams to keep patients safe and comfortable at home.

Ideal candidates hold an active Minnesota CNP license (AGNP preferred), with experience in wound care, geriatrics, or post-acute care, and willingness to travel throughout

Qualifications

• Active Minnesota CNP license required; AGNP preferred.
• Experience in wound care, geriatrics, primary care, post-acute care, or skilled nursing preferred.

Responsibilities

• Provide advanced wound care assessments, treatment planning, and management of complex wounds.
• Conduct rounding visits in senior living communities (assisted living, memory care, skilled nursing, long-term care).
• Manage medications, diagnostics, treatment plans, and follow-up care.
• Collaborate with physicians, nurses, therapists, caregivers, and community staff.
• Support interventions to improve healing and reduce complications.
• Help patients maintain high quality of life throughout their care journey.

Compensation: $120,000 to $160,000+ Total Compensation (Base Salary + Productivity Incentive Program) + Mileage Reimbursement

What You'll Be Doing

• Provide advanced wound care assessments, treatment planning, and ongoing management of complex wounds.
• Conduct routine rounding visits throughout assisted living, memory care, skilled nursing, and long-term care communities.
• Manage medications, diagnostics, treatment plans, and follow-up care.
• Collaborate with physicians, nurses, therapists, caregivers, and community staff.
• Support proactive interventions that improve healing outcomes and reduce avoidable complications.
• Help patients maintain the highest possible quality of life throughout their healthcare journey.

Qualifications

• Active Minnesota Certified Nurse Practitioner (CNP) license (AGNP preferred).
• Experience in wound care, geriatrics, primary care, post-acute care, or skilled nursing preferred.
• Strong clinical judgment and confidence managing medically complex patient populations.
• Valid driver's license and ability to travel throughout the Twin Cities Metro Area.
