Job summary

Twin Cities Physicians seeks a full-time Nurse Practitioner (AGNP/CNP) to deliver advanced wound care across senior living communities in the Twin Cities Metro. You’ll provide clinical support where patients live, manage complex wounds, and collaborate with a multidisciplinary team to improve healing and quality of life.

The role emphasizes compassionate care, ongoing assessment, and partnerships with families and facilities to keep residents healthier and safer.

Qualifications

• Active Minnesota CNP license (AGNP preferred).
• Experience in wound care, geriatrics, primary care, or post-acute care.
• Strong clinical judgment for complex patients.
• Valid driver's license and ability to travel across the Twin Cities Metro.

Responsibilities

Compensation: $120,000 to $160,000+ Total Compensation (Base Salary + Productivity Incentive Program) + Mileage Reimbursement
