Twin Cities Physicians is seeking a full-time Wound Care Nurse Practitioner (AGNP/CNP) to deliver advanced wound care in assisted living and memory care communities across the Twin Cities Metro. This on-site role offers a competitive compensation package with base salary plus productivity incentives, mileage reimbursement, full benefits, PTO, CME allowance, malpractice coverage, and DEA reimbursement.

Qualifications

• Active Minnesota CNP license.
• Experience in wound care, geriatrics, primary care, post-acute or related settings preferred.
• Strong clinical judgment and ability to manage medically complex patients.
• Valid driver’s license and ability to travel throughout the Twin Cities Metro Area.

Responsibilities

• Provide advanced wound care assessments, treatment planning and ongoing management in senior living communities.
• Conduct routine rounding visits and follow-up care across assigned communities.
• Manage medications, diagnostics, treatment plans and clinical interventions.
• Collaborate with physicians, nurses, therapists, caregivers and community staff.
• Identify risks early and implement strategies to prevent complications and hospitalizations.

Schedule: Full-Time | Monday through Friday
