PHP Nurse Practitioners have a passion for delivering quality care to seniors and other residents in healthcare settings such as skilled nursing facilities, assisted living, independent living communities, and via telehealth.

We are currently recruiting Nurse Practitioners at Taylor Healthcare Center in Grafton, WV, working Monday to Friday, 8 am to 5 pm. This is a salaried position with some weeks exceeding 40 hours, but no weekend or holiday coverage is required. The role offers ongoing educational opportunities and a supportive environment.

Job Summary

The PHP NP provides high-quality primary care in a person-centered model, diagnosing, treating, and managing acute and chronic illnesses, collaborating with an interdisciplinary team, and engaging residents and families in care planning.

Job Duties & Responsibilities

• Assess, diagnose, and manage conditions.
• Provide health education and develop treatment plans.
• Order and interpret tests, document within EMR, and collaborate with care teams.
• Support strategic initiatives and ensure high customer satisfaction.
• Practice ethically, comply with regulations, and participate in care coordination and family conferences.

Minimum Qualifications

• Master's degree in Nursing or equivalent, with national certification (AANP or ANCC).
• Active, unrestricted state license and DEA license.
• At least 2 years of clinical experience in geriatric, adult, or family practice.
• Ability to work flexibly and a passion for senior and low-income populations.

Preferred Qualifications

• Geriatrics training or experience, 2+ years as a Nurse Practitioner.
• Experience with ICD-10, HCC coding, and value-based care models.
