PHP Nurse Practitioners have a passion for delivering quality care to seniors and other residents in various healthcare settings, including skilled nursing facilities, assisted living, independent living communities, and via telehealth.

PHP is currently recruiting Nurse Practitioners at Holbrook Healthcare Center in Buckhannon, WV. Work Monday - Friday, 8a-5p, salaried, with some weeks exceeding 40 hours. No call, weekends, or holiday coverage. Must be willing to work 5 days a week, Monday - Friday.

Job Summary:

The PHP NP provides high-quality primary care in a person-centered model, diagnosing, treating, and managing acute and chronic illnesses, collaborating with the care team, and engaging residents and families in care planning to reduce hospitalizations and ER visits.

Job duties & responsibilities:

• Assess, diagnose, and manage conditions.
• Provide health education.
• Develop and communicate treatment plans.
• Order and interpret tests, adjust treatments.
• Maintain compliant documentation in EMR.
• Collaborate with leadership and support strategic initiatives.
• Ensure excellent customer experience and satisfaction.
• Practice ethically, adhering to legal and professional standards.
• Follow all regulations and laws.
• Coordinate care with other providers, including telemedicine.
• Complete required visits and participate in family conferences.
• Address pharmacy recommendations and ensure face-to-face coverage in facilities.

Qualifications & Experience:

Required:

• MSN, DNP, or NP Certificate.
• National certification (AANP or ANCC).
• Active, unrestricted state license.
• Active DEA license.
• US work authorization.
• 2+ years clinical experience in geriatrics, adult, or family practice.
• Flexible schedule ability.
• Passion for senior and low-income healthcare.

Preferred:

• Geriatrics training or experience.
• 2+ years NP experience.
• Experience with ICD-10, HCC coding.
• Experience in metrics-driven healthcare models.
• Willingness for professional development.
• Experience in value-based care, outpatient primary care, or skilled nursing settings.
