PHP Nurse Practitioners have a passion for delivering quality care to seniors and other residents in various healthcare settings, including skilled nursing facilities, assisted living, independent living communities, and via telehealth.

PHP is recruiting Nurse Practitioners for the Berea Alzheimer's Care Center in Berea, OH. Work hours are Monday - Friday, 8a-5p, salaried, with some weeks exceeding 40 hours. No call, weekends, or holiday coverage required. Must work 5 days a week, Monday - Friday.

Job Summary:

The PHP NP provides high-quality primary care in a person-centered model, diagnosing, treating, and managing acute and chronic illnesses, working collaboratively with a healthcare team, and involving residents and families in care planning. This model aims to reduce hospital and ER visits, focusing on customer satisfaction.

Job duties & responsibilities:

1. Assess, diagnose, and manage conditions.
2. Provide health education.
3. Develop and communicate treatment plans.
4. Order and interpret tests, adjust treatments.
5. Maintain compliant documentation in EMR.
6. Collaborate with clinical leadership and support strategic initiatives.
7. Ensure excellent patient experience and satisfaction.
8. Practice ethically, adhering to scope, standards, and laws.
9. Coordinate care with other providers and telemedicine.
10. Complete regulatory visits and participate in family conferences.
11. Address pharmacy and medication recommendations.

Qualifications & Experience:

Required:

• Master of Science in Nursing, DNP, or NP Certificate.
• National certification (AANP or ANCC).
• Active, unrestricted state license.
• Active DEA license.
• US work authorization.
• 2+ years clinical experience in geriatrics, adult, or family practice.
• Flexible schedule and environment.
• Passion for senior and low-income healthcare.

Preferred:

• Geriatrics training or experience.
• 2+ years as a Nurse Practitioner.
• Experience with coding, ICD-10, HCC.
• Experience in results-driven healthcare models.
• Willingness for professional development.
• Experience in value-based care, assisted living, SNF, outpatient primary care.
