PHP Nurse Practitioners are dedicated to delivering quality care to seniors and residents in various healthcare settings, including skilled nursing facilities, assisted living, independent living communities, and via telehealth.

PHP is recruiting Nurse Practitioners for our facility at Taylor Healthcare Center in Grafton, WV. The position involves working Monday to Friday, 8 am to 5 pm, with a salaried pay structure. Some weeks may require more than 40 hours. No call, weekends, or holiday coverage is required. Applicants must be willing to work five days a week, Monday through Friday.

The role offers a supportive environment with ongoing educational opportunities in disease management, palliative care, and best practices in SNF care.

Job Summary:

The PHP NP provides high-quality primary care in a person-centered care model, diagnosing, treating, and managing acute and chronic illnesses through wellness visits, early detection, and timely treatment. The NP collaborates with primary care providers and interdisciplinary teams, involving residents and families in care planning. This approach aims to reduce hospital admissions and ER visits, emphasizing customer satisfaction.

Job duties & responsibilities:

• Assess, diagnose, and manage conditions.
• Provide health education.
• Develop and communicate treatment plans.
• Order and interpret tests, adjust treatments accordingly.
• Maintain compliant documentation in EMR.
• Collaborate with clinical and operational leadership.
• Support value-based care initiatives.
• Ensure excellent customer experience.
• Practice ethically per scope and standards, following regulations.
• Coordinate care with other providers, including telemedicine.
• Complete required visits and participate in family conferences.
• Address pharmacy and medication communications.

Qualifications & Experience:

Required:

• MSN, DNP, or NP certification graduate.
• National certification (AANP or ANCC).
• Active, unrestricted state license.
• Active DEA license.
• US work authorization.
• 2+ years clinical experience in geriatrics, adult, or family practice.
• Flexible schedule and environment.
• Passion for senior and low-income healthcare.

Preferred:

• Geriatrics training or experience.
• 2+ years as a Nurse Practitioner.
• Experience with ICD-10, HCC coding.
• Experience in results-driven healthcare models.
• Willingness for ongoing professional development.
• Experience in value-based care, outpatient primary care, or skilled nursing settings.
