PHP Nurse Practitioners are passionate about delivering quality care to seniors and residents in various healthcare settings, including skilled nursing facilities, assisted living, independent living communities, and via telehealth.

We are currently recruiting Nurse Practitioners at The Pines Healthcare Center in Canton, OH. Our Nurse Practitioners work Monday - Friday, 8 am - 5 pm, with a salaried position, no call, weekends, or holiday coverage. Some weeks may exceed 40 hours. Must be willing to work five days a week, Monday through Friday.

Job Summary:

The PHP Nurse Practitioner provides high-quality primary care in a person-centered care model, diagnosing, treating, and managing acute and chronic illnesses, and collaborating with an interdisciplinary team.

Key Responsibilities:

1. Assess, diagnose, and manage conditions.
2. Provide health education.
3. Develop and communicate treatment plans.
4. Order and interpret diagnostic tests.
5. Maintain compliant documentation in EMR.
6. Collaborate with the care team and support strategic initiatives.
7. Ensure excellent patient experience and satisfaction.
8. Follow legal and ethical standards.
9. Coordinate with other providers and complete regulatory visits.
10. Participate in family conferences and advance care planning.

Qualifications & Experience:

Required:

• Master's or DNP degree or NP certification.
• Active certification (AANP or ANCC).
• Unrestricted state license and DEA license.
• US work authorization.
• 2+ years clinical experience in geriatrics, adult, or family practice.
• Flexible schedule and strong passion for senior healthcare.

Preferred:

• Geriatrics training or experience.
• Experience with ICD-10, HCC coding, and value-based care.
• Experience in outpatient primary care or collaborative healthcare settings.
