PHP Nurse Practitioners have a passion for delivering quality care to seniors and other residents in various healthcare settings, including skilled nursing facilities, assisted living, independent living communities, and via telehealth.

PHP is recruiting Nurse Practitioners to provide high-quality care at Bridgewater Healthcare Center in Carmel, IN. Work Monday - Friday, 8a-5p, with a salaried position, no call, weekends, or holiday coverage. Must be willing to work five days a week.

Job Summary

The PHP NP provides high-quality primary care in a person-centered model, diagnosing, treating, and managing acute and chronic illnesses, collaborating with the care team, and involving residents and families in care plans.

Job Duties & Responsibilities

• Assess, diagnose, and manage conditions.
• Provide health education.
• Develop and communicate treatment plans.
• Order and interpret diagnostic tests.
• Maintain compliant documentation in EMR.
• Collaborate with leadership and support strategic initiatives.
• Ensure excellent customer experience and satisfaction.
• Practice ethically per standards and regulations.
• Coordinate with care teams and telemedicine providers.
• Complete regulatory visits and participate in family conferences.
• Ensure face-to-face coverage in facilities.

Required Qualifications & Experience

• MSN, DNP, or NP Certificate, with national certification (AANP or ANCC).
• Active, unrestricted license and DEA license.
• US work authorization.
• 2+ years of clinical experience in geriatrics, adult, or family practice.
• Flexible schedule and commitment to senior and low-income populations.

Preferred Qualifications

• Geriatrics training or experience.
• 2+ years as a Nurse Practitioner.
• Experience with coding, ICD-10, HCC.
• Experience in value-based care models and outpatient primary care.
