Nurse Practitioner, Adult and/or Gerontology
PHP Nurse Practitioners are passionate about delivering quality care to seniors and other residents in various healthcare settings, including skilled nursing facilities, assisted living, independent living communities, and via telehealth.
PHP is currently recruiting Nurse Practitioners who are committed to providing high-quality care at Evergreen Crossing and The Lofts in Indianapolis, IN. Our Nurse Practitioners work Monday through Friday, from 8 am to 5 pm. They are salaried, with some weeks exceeding 40 hours. No call, weekends, or holiday coverage required. Must be available to work five days a week, Monday to Friday.

The PHP Nurse Practitioner provides high-quality primary care in a person-centered care model. Responsibilities include diagnosing, treating, and managing acute and chronic illnesses through wellness visits, early detection, and timely intervention. The NP collaborates with primary care providers and interdisciplinary teams, involving residents and families in care planning, aiming to reduce hospitalizations and ER visits while enhancing customer satisfaction.
