Nurse Practitioner (NP), Adult and/or Gerontology, Homestead Healthcare Center

Position Overview

PHP Nurse Practitioners are dedicated to delivering quality care to seniors and residents in various healthcare settings, including skilled nursing facilities, assisted living, independent living communities, and via telehealth.

We are currently recruiting Nurse Practitioners passionate about providing high-quality care at Homestead Healthcare Center in Indianapolis, IN. Work hours are Monday - Friday, 8 am - 5 pm, salaried, with some weeks exceeding 40 hours. No call, weekends, or holiday coverage required. Must be willing to work five days a week.

Responsibilities

• Assess, diagnose, and manage chronic and acute conditions.
• Provide health education to residents and families.
• Develop and communicate treatment plans effectively.
• Order, interpret lab and diagnostic tests, and adjust treatments accordingly.
• Maintain compliant documentation within our EMR system.
• Collaborate with healthcare teams and support strategic initiatives in value-based care.
• Ensure excellent patient experience and satisfaction.
• Follow all regulatory and ethical standards.
• Coordinate care with other providers, including telemedicine providers.
• Complete required visits and participate in family conferences and advanced care planning.

Minimum Qualifications

• Master of Science in Nursing, DNP, or NP Certificate.
• National certification (AANP or ANCC).
• Active, unrestricted state license and DEA license.
• US work authorization.
• At least 2 years of clinical experience in geriatric, adult, or family practice.

Preferred Qualifications

• Geriatrics training or clinical experience.
• Over 2 years of NP clinical experience.
• Experience with ICD-10 coding, HCC, and results-driven healthcare models.
• Experience in outpatient primary care and value-based care settings.
