PHP is currently recruiting Nurse Practitioners who are passionate about delivering high-quality care to seniors and residents in various healthcare settings, including skilled nursing facilities, assisted living, independent living communities, and via telehealth.

Position Details:

• Location: Bridgewater Healthcare Center, Carmel, IN
• Schedule: Monday - Friday, 8 am - 5 pm; salaried; some weeks may exceed 40 hours; no call, weekends, or holidays

Job Summary:

The PHP Nurse Practitioner provides high-quality primary care in a person-centered model, managing acute and chronic illnesses, conducting wellness visits, and collaborating with the healthcare team and families to develop care plans. This approach aims to reduce hospital and ER visits and enhance patient satisfaction.

Responsibilities include:

1. Assessing, diagnosing, and managing conditions
2. Providing health education
3. Developing and communicating treatment plans
4. Ordering and interpreting tests
5. Documenting in EMR
6. Collaborating with providers and care teams
7. Supporting value-based care initiatives
8. Ensuring regulatory compliance
9. Participating in family conferences and care planning
10. Maintaining face-to-face coverage in facilities

Qualifications & Experience:

• Master's in Nursing, DNP, or NP certification
• Active, unrestricted state license and DEA license
• US work authorization
• At least 2 years of clinical experience in geriatrics, adult, or family practice
• Ability to work flexibly and a passion for senior and low-income populations

Preferred:

• Geriatrics training or experience
• Experience with ICD-10, HCC coding, and value-based care models
• Experience in outpatient primary care or skilled nursing settings
