Nurse Practitioner (NP), Adult and/or Gerontology, Evergreen Crossing and The Lofts

Join us at CommuniCare Health Services as a Nurse Practitioner (NP), Adult and/or Gerontology at Evergreen Crossing and The Lofts in Indianapolis, IN.

Position Overview

PHP Nurse Practitioners are dedicated to providing high-quality care to seniors and residents across various healthcare settings, including skilled nursing facilities, assisted living, independent living communities, and telehealth. Our Nurse Practitioners work Monday through Friday, 8 am to 5 pm, with no weekend or holiday coverage, offering a supportive environment with ongoing educational opportunities in disease management, palliative care, and best practices in SNF care.

Responsibilities

• Assess, diagnose, and manage chronic and acute conditions.
• Provide health education and develop treatment plans.
• Order and interpret lab and diagnostic tests, adjusting treatment as needed.
• Maintain compliant documentation within our EMR system.
• Collaborate with the healthcare team and support strategic initiatives in value-based care.
• Ensure excellent patient experience and satisfaction.
• Participate in family conferences and advanced care planning.
• Coordinate care with other providers, including telemedicine services.

Minimum Qualifications

• Master of Science in Nursing, DNP, or NP Certification.
• Active certification (AANP or ANCC) and unrestricted state license.
• DEA license and US work authorization.
• At least 2 years of clinical experience in geriatric, adult, or family practice.

Preferred Qualifications

• Geriatrics training or experience.
• Experience with ICD-10, HCC, and value-based care models.
• Experience in outpatient primary care or skilled nursing settings.
