Job summary

Premise Health is seeking a Full-Time Nurse Practitioner (NP) to provide care to employees and dependents at our Cheraw Health Center in Cheraw, SC. The role offers a Monday–Friday schedule from 7:30 AM to 4:30 PM with no weekends, a $5,000 sign-on bonus, and opportunities for CME allowance and growth.

You will conduct exams, develop care plans, collaborate with physicians, and educate patients on treatment plans and preventive care within a primary care setting.

Qualifications

• Active NP license to practice in SC.
• Current national certification (ANCC or AANP).
• Prescriptive authority where applicable.
• Minimum three years of clinical NP experience.

Responsibilities

• Perform physical exams and preventive health measures per guidelines.
• Develop patient care plans, record progress, and ensure continuity of care.
• Prescribe or recommend treatments, including medications and therapies.
• Collaborate with physicians and healthcare teams for comprehensive care.
• Educate patients on treatment plans and health maintenance.

What You’ll Do

• Conduct physical exams and preventive health measures per guidelines
• Perform procedures such as injections, suturing, wound care, and infection management
• Develop patient care plans, record progress, and ensure continuity of care
• Educate patients on treatment plans, health maintenance, and development
• Order and interpret diagnostic tests to assess clinical conditions
• Collaborate with physicians and healthcare teams for comprehensive care
• Prescribe or recommend treatments, including medications and therapies
• Refer patients to specialists or physicians as needed
• Maintain patient health records ensuring accurate and up-to-date documentation
• Educate and coach nursing staff on best practices
• Perform other duties as assigned

What You’ll Bring

• Maintain state RN licenses/certificates to practice
• Maintain Nurse Practitioner licenses/certificates to practice
• Maintain State Prescriptive Authority (if applicable)
• Federal DEA license (matching each state of practice as applicable)
• State Narcotic/Controlled Substance license if required in your state
• Current national certification (ANCC or AANP required)
• Master’s Degree in Science of Nursing (if Master Degree in Science certificate does not state Nursing on it then a copy of transcripts is required)
• Current certification with hands‑on training in AHA, ARC or equivalent Basic Life Support for health care providers is required; Advanced Cardiac Life Support may also be required based on contract scope of services
• At least three years of clinical experience as a Nurse Practitioner in an ambulatory care, occupational health, family practice or emergency department setting
