Job summary

GuideStar Eldercare in Minnesota is seeking a Nurse Practitioner to join our multidisciplinary team. You will address dementia and neurocognitive care in nursing homes and assisted living facilities, enjoying the autonomy of private practice with a supported caseload and administrative backup.

You will travel daily from your home to assigned facilities, perform initial evaluations, ongoing therapy, and cognitive testing, document needs, and build strong relationships with patients, families, and

Qualifications

Proof of State licensure in Minnesota.
Pass pre-employment background check and drug screening.
Complete credentialing process.

Responsibilities

Travel daily to assigned facilities from home to provide care.
Provide initial evaluations and ongoing therapy services, including cognitive testing.
Document the need and extent of services for each patient.
Create and maintain positive professional relationships with facilities, patients, and families.
