Job summary

GuideStar Eldercare seeks a Nurse Practitioner to address neurobehavioral needs in dementia care within nursing homes and assisted living facilities. Travel daily from your home to assigned sites, with support from clinical leadership and in-house scheduling.

You will perform evaluations, therapy, and cognitive testing while coordinating care with families and facility staff. You'll join a multidisciplinary team dedicated to safety, dignity, and functional outcomes, with a structured onboarding

Qualifications

• Proof of Nurse Practitioner licensure.
• Ability to work as part of an interdisciplinary treatment team.
• Good time management skills.
• Able to function autonomously with regional Area Manager support.
• Pass pre-employment background check and drug screening.
• Complete credentialing process.

Responsibilities

• Travel daily to assigned facilities from your home.
• Provide initial evaluations, continuing therapy services, and cognitive testing.
• Document the need and extent of services for each patient.
• Create and maintain positive professional relationships with client facilities, patients, and their families.
