GuideStar Eldercare is seeking a Nurse Practitioner to serve in a neurobehavioral long-term care setting across nursing homes and assisted living facilities in Indiana. The role combines private practice autonomy with corporate clinical leadership support and in-house scheduling, coding, and billing.

You will travel daily from home to assigned facilities to perform initial evaluations, provide ongoing therapy, and conduct cognitive testing, documenting need and service extent while building

Qualifications

• Indiana state licensure for NP
• Pre-employment background check and drug screening
• Credentialing process completion

Responsibilities

• Travel daily to assigned facilities from your home.
