Job summary

Oak Street Health is seeking a Nurse Practitioner to deliver effective, value-based primary care to adults on Medicare. You will see patients independently within a collaborative care team, conduct wellness visits, and engage in care coordination with physicians, nurses, and specialists to optimize patient health.

Responsibilities include office visits for routine and acute issues, health risk assessments, patient education, and participation in outreach activities.

Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or NP certificate program.
• Active, non-probationary NP license required.
• Active DEA license required.
• US work authorization required.

Responsibilities

• Provide effective and equitable value-based primary care to adults on Medicare.
• Conduct office visits for routine and acute issues.
• Perform annual wellness visits and health risk assessments with thorough documentation.
• Coordinate care with physicians, specialists, and care team members.
• Lead population health initiatives with the care team.
• Participate in triage, outreach, and patient education.
• Engage in promotional activities and community outreach.
• Possibly perform home visits as needed.
