Palm Primary Care in Texas delivers value-based, quality healthcare for seniors and families, with a 98% patient satisfaction rate and centers staffed by caring professionals. We provide comprehensive primary care in welcoming centers designed to be comfortable and accessible for our communities.

We offer a Monday–Friday in-person schedule with no on-call, a supportive team, and relocation assistance for qualified candidates.

Qualifications

• Have at least 2-3 years' experience in the primary care/value-based care setting, working with Medicare Advantage, Medicare, and Medicaid patient populations
• Must have an active and unrestrictive TexasAPRN license
• Must have DEA license or eligible to obtain it within 2 months of date of hire
• Strong understanding of chronic disease management, preventative care, and quality metrics in a value-based care model
• Excellent documentation and patient relationship-building skills
• Proficient with EMR systems (eCW a plus!)
