Job summary

Palm Medical Centers delivers senior primary care with a focus on value-based care and compassionate service. Our centers provide primary care and other services to seniors and families in comfortable, convenient medical centers staffed by caring professionals.

We offer a Monday–Friday schedule with great work/life balance for providers (no on-call). This in-person role includes benefits such as health, dental, vision, 401(k), and paid time off, plus relocation assistance and a retirement plan.

Qualifications

• 2–3 years of experience in primary care/value-based care with Medicare Advantage, Medicare, and Medicaid populations.
• Active TexasAPRN license with unrestricted status.
• DEA license or eligible to obtain within 2 months of date of hire.
• Strong understanding of chronic disease management, preventative care, and quality metrics in a value-based care model.
• Excellent documentation and patient relationship-building skills.
• Proficient with EMR systems (eCW a plus)

Qualifications:

• Have at least 2-3 years' experience in the primary care/value-based care setting, working with Medicare Advantage, Medicare, and Medicaid patient populations
• Must have an active and unrestrictive TexasAPRN license
• Must have DEA license or eligible to obtain it within 2 months of date of hire
• Strong understanding of chronic disease management, preventative care, and quality metrics in a value-based care model
