A leading healthcare provider in Texas is seeking a dedicated professional to deliver quality primary care to senior patients. Qualifications

• DEA license or eligibility to obtain it within 2 months of hire.
• Strong understanding of chronic disease management, preventative care, and quality metrics in a value-based care model
• Excellent documentation and patient relationship-building skills
• Proficient with EMR systems (eCW a plus!)

Qualifications:

• Have at least 2-3 years' experience in the primary care/value-based care setting, working with Medicare Advantage, Medicare, and Medicaid patient populations
• Must have an active and unrestrictive Florida APRN license
