Job summary

Palm Primary Care, part of Palm Medical Centers, delivers senior primary care with a patient-first approach in a in-person setting. The team emphasizes value-based care and quality metrics, serving seniors and families in our comfortable centers with caring professionals.

The role focuses on comprehensive primary care, chronic disease management, and strong patient relationships, with a Monday–Friday schedule and no on-call. We offer relocation assistance and a robust benefits package.

Qualifications

• 2–3 years in primary care/value-based care with Medicare Advantage/Medicaid populations.
• Active Texas APRN license in good standing.
• DEA license or eligible to obtain within 2 months of hire.
• Strong chronic disease management and quality metrics knowledge in value-based care.
• Excellent documentation and patient relationship-building skills.
• Proficient with EMR systems (eCW a plus).

Qualifications

• Have at least 2-3 years' experience in the primary care/value-based care setting, working with Medicare Advantage, Medicare, and Medicaid patient populations
• Must have an active and unrestrictive Texas APRN license
• Must have DEA license or eligible to obtain it within 2 months of date of hire
• Strong understanding of chronic disease management, preventative care, and quality metrics in a value-based care model
• Excellent documentation and patient relationship-building skills
• Proficient with EMR systems (eCW a plus!)
