Job summary

Palm Primary Care is seeking an experienced APRN to join our in-person team in Texas. You will provide primary care services within a value-based care model, manage chronic conditions, and collaborate with a multidisciplinary staff to improve patient outcomes.

Qualifications

• Active Texas APRN license in good standing.
• Experience in primary care within value-based care models.
• DEA license or eligibility to obtain within 2 months.
• Strong documentation and patient relationship skills.
• Proficient in EMR systems; eCW familiarity a plus.

Job description

Qualifications:

• Have at least 2-3 years' experience in the primary care/value-based care setting, working with Medicare Advantage, Medicare, and Medicaid patient populations
• Must have an active and unrestrictive TexasAPRN license
• Must have DEA license or eligible to obtain it within 2 months of date of hire
• Strong understanding of chronic disease management, preventative care, and quality metrics in a value-based care model
• Excellent documentation and patient relationship-building skills
• Proficient with EMR systems (eCW a plus!)
