Palm Medical Centers is seeking a Primary Care Nurse Practitioner to join our premier team in Florida. We focus on value-based, patient-centered care and helping patients stay healthy.

Qualifications

• 2–3 years' experience in primary care/value-based care with Medicare Advantage/Medicaid.
• Active Florida APRN license with DEA eligibility.
• DEA license or eligible within 2 months of hire.

Responsibilities

• Maintain accurate documentation and build strong patient relationships.
• Collaborate with a care team to coordinate patient care.

Job Requirements:

• Have at least 2-3 years' experience in the primary care/value-based care setting, working with Medicare Advantage, Medicare, and Medicaid patient populations
• Must have an active and unrestrictive Florida APRN license
• Must have DEA license or eligible to obtain it within 2 months of date of hire
• Strong understanding of chronic disease management, preventative care, and quality metrics in a value-based care model
• Excellent documentation and patient relationship-building skills
• Proficient with EMR systems (eCW a plus!)
