Job summary

Palm Primary Care in Texas is seeking an experienced APRN to provide senior primary care within a value-based care framework. You will deliver comprehensive care in our in-person centers, focusing on chronic disease management and preventive care for Medicare/Medicaid populations.

The role requires an active Texas APRN license, a DEA license or eligibility, and strong documentation and patient relationship skills. Monday to Friday schedule offers a balanced workload and no on-call obligations.

Qualifications

• Active Texas APRN license with no restrictions.
• DEA license or eligibility within 2 months of hire.
• 2–3 years’ experience in primary care/value-based care with Medicare/Medicaid populations.
• Strong understanding of chronic disease management and quality metrics in a value-based care model.
• Excellent documentation and patient relationship-building skills.
• Proficient with EMR systems; ECW a plus.

• Collaborate with care teams to manage chronic conditions and preventive care.
• Maintain accurate patient records and documentation.
• Engage in community programs and events where applicable.

Qualifications

• Have at least 2-3 years’ experience in the primary care/value-based care setting, working with Medicare Advantage, Medicare, and Medicaid patient populations
• Must have an active and unrestrictive TexasAPRN license
• Must have DEA license or eligible to obtain it within 2 months of date of hire
• Strong understanding of chronic disease management, preventative care, and quality metrics in a value-based care model
• Excellent documentation and patient relationship-building skills
• Proficient with EMR systems (eCW a plus!)
