Qualifications

• Active Florida APRN license required.

Responsibilities

• Provide primary care services in a value-based care model.
• Manage chronic diseases and preventive care for geriatrics and adults.
• Educate patients and coordinate with an interdisciplinary team.
• Document thoroughly in the EMR and ensure quality metrics are met.

Job Requirements:

• Have at least 2-3 years' experience in the primary care/value-based care setting, working with Medicare Advantage, Medicare, and Medicaid patient populations
• Must have an active and unrestrictive Florida APRN license
• Must have DEA license or eligible to obtain it within 2 months of date of hire
• Strong understanding of chronic disease management, preventative care, and quality metrics in a value-based care model
• Excellent documentation and patient relationship-building skills
• Proficient with EMR systems (eCW a plus!)
