Job summary

Longevity Health in Laredo, TX seeks a Nurse Practitioner or Physician Assistant to manage a panel of nursing home members, supported by a clinical leadership team. You will perform comprehensive assessments and develop individualized care plans, while communicating frequently with families and the care team to align with each member's goals.

Role requires 6+ months of NP/PA experience, an MSN/NP degree or equivalent, and strong clinical skills.

Qualifications

• 6+ months experience as Nurse Practitioner or Physician Assistant.
• MSN as NP or master's degree from a nationally accredited program.
• Experience in nursing home environment preferred.
• Computer/technology proficiency required.
• Effective communication with physicians, members and families.
• Self-directed and team-oriented with strong clinical skills.

Responsibilities

• Provides collaborative primary care to a panel of members.
• Delivers ISNP Model of Care activities and advanced care planning.
• Maintains communications with PCPs, families and care team on care plans.
• Participates in facility care team meetings and coordinates care.
• Ensures alignment with members' goals and needs.
• Supports market goals and balances member caseloads.
