Qualifications

• Licensed NP or PA in state of operation with current board certification.
• Master's degree in Nursing or PA studies.
• Strong leadership, interpersonal, and problem-solving skills.
• Excellent written and oral communication.

Responsibilities

• Perform comprehensive health assessments and develop care plans for residents in skilled nursing, assisted living, and independent living.
• Diagnose and manage acute and chronic conditions with a focus on prevention.
• Provide ongoing care management to improve quality of life.
• Conduct facility rounds and build relationships with residents, families, and care teams.
• Coordinate care with physicians, staff, and specialists.
• Maintain EMR documentation and ensure regulatory compliance.
