Your Health Organization in Georgia is seeking a Nurse Practitioner to join our growing primary care team. The Provider will perform visits in patients' homes and facilities across Athens, Elberton, and Hartwell, with daily travel required.

This full-time, salary-based role offers bonus opportunities and a collaborative environment to coordinate comprehensive care, partner with families and other professionals, and deliver high-quality, patient-centered care across settings.

Qualifications

• Licensed Nurse Practitioner in the state of employment with good standing.
• Experience in home health, clinic, facility, telehealth, or specialty care.
• Strong communication and interpersonal skills.
• Ability to evaluate high-risk patients and develop care plans.
• Willingness to travel daily and work in homes and facilities.

Responsibilities

• Evaluate high-risk patients and develop individualized care plans.
• Coordinate care with interdisciplinary team to ensure continuity.
• Communicate with case managers, clinicians, pharmacists, and others.
• Participate in interdisciplinary team meetings to review progress.
• Document care management activities accurately for regulatory compliance.
• Educate patients and families on conditions and self-management.
