Job summary

Your Health Organization in Georgia is seeking a Nurse Practitioner to deliver patient care in homes, clinics, and facilities across Athens, Elberton, and Hartwell. This is a full-time role requiring reliable transportation and daily travel within the designated service area.

The Provider will work with an interdisciplinary team to evaluate high-risk patients, coordinate care plans, and ensure ongoing care management.

Qualifications

• Nurse Practitioner, Physician Assistant, or Physician license in good standing.
• Experience in home health, clinic, facility, telehealth or specialty care setting preferred.
• Strong communication, collaboration and interpersonal skills.
• Ability to evaluate high-risk patients, develop care plans, and coordinate care.
• Willingness to travel between offices and service areas.
• Commitment to patient-centered care and HIPAA compliance.

Responsibilities

• Evaluate high-risk patients and develop individualized care plans.
• Coordinate care activities with interdisciplinary team.
• Communicate with case managers, clinicians, pharmacists, and other team members.
• Participate in interdisciplinary team meetings to review patient progress.
• Document care management activities accurately for compliance.
• Provide patient education and resources for self-management.
• Travel to patients' homes and facilities as required.
