Your Health Organization in Georgia seeks a Nurse Practitioner to deliver patient care across Athens, Elberton, and Hartwell. This full-time role involves home visits, facilities, telehealth, and clinics, with daily travel required and a monthly bonus opportunity.

The provider will evaluate high-risk patients, coordinate care, and educate patients and families. A valid NP/PA/Physician license in good standing is required, with strong communication and teamwork skills expected.

Qualifications

• NP/PA/Physician license in good standing.
• Experience in home health, clinic, facility, telehealth, or specialty care.
• Strong communication and interpersonal skills.
• Ability to evaluate high-risk patients and coordinate care.
• Must be a licensed driver with reliable transportation.

Responsabilités

• Evaluate high-risk patients and develop individualized care plans.
• Coordinate care activities with interdisciplinary team for continuity.
• Communicate with case managers, clinicians, pharmacists, and health coaches.
• Document care management activities for regulatory compliance.
• Provide patient education and self-management resources.
• Travel daily to patient homes, clinics and facilities.
